The Staffordshire and Stoke-on-Trent Partnership Trust is the part of the NHS that provides health and care services in your community including community hospitals, health centres, nursing homes, schools, prisons and in your own home. Our teams include district nursing, occupational therapy, dental services, school nursing, health visitors, sexual health and lifestyle services, physiotherapy, hospital nursing and many more.

From April 1^st 2012, the Partnership Trust now also provides adult social care services for people in Staffordshire, (outside Stoke-on-Trent) giving people access to a one-stop service for all their health and care needs.
