The Clinical Nurse Specialist service supports people with Mild, Moderate and Severe Learning Disabilities to access mainstream health care and ensure health professionals gain knowledge and skills in being able to provide primary healthcare that is responsive to their individual needs; to reduce health inequalities.

Littleton Intermediate Care
GP, Nurse, Therapy and Social Care led unit. Aims to reduce admissions, readmission rates, delayed discharges and excess bed days in relation to acute care. Also facilitates the safe and timely discharge patients following an acute in-patient episode and to prevent medical and functional decline which may result in the patient requiring an acute inpatient admission.
Long Term Conditions
The Partnership Trust’s Long Term Conditions Team is a multi-disciplinary team working in CHD, Diabetes, Respiratory Disease, Learning Disabilities, Osteoporosis and Falls. Many team members hold a caseload, the role is split between direct patient care and supporting other Trust staff that are delivering these services e.g. physiotherapists.
Longton Cottage Community Hospital
Longton Cottage Hospital provides intermediate care and rehabilitation to patients prior to discharge from home after treatment at an Acute Hospital; patients may also be discharged to nursing homes.
Lymphoedema Service
This service covers the Seisdon area, providing treatment and advice for patients with Lymphoedema within clinics and on a domiciliary basis.
