Information for families living in Stoke-on-Trent who access health visiting service. 

Health Visiting - Stoke-on-Trent

Health visiting teams work within the community to support local children and families. In Stoke-on-Trent health visiting teams provide the Healthy Child Programme for all families with children up to the age of five (5). The programme ensures a healthy start for children, (for example immunisation, health and development checks), support for parents and access to a range of resources.

In Stoke-on-Trent all families will receive the universal offer, which includes: 

  • Contact from your Health Visiting team at around 28 weeks of pregnancy.
  • A new born visit 10–14 days after the baby is born.
  • A mother and baby review when your baby is 5-8 weeks old.
  • A mother and baby review when your baby is 3-4 months old.
  • A 9–12 months old review.
  • A 2 year–2 and a half years review.

Some families may require more support. Your health visitor will help identify the level of support which your family requires and if needed, will talk you through additional offers of support available to you.
