Our Trust Board
A Board of Directors are responsible for running the organisation. They include four directors who are members of the community as well as directors who work full time for us.

Tripartite Formal Agreement
All NHS Trusts are required to achieve Foundation Trust status by 1 April 2014. Becoming a Foundation Trust gives an NHS Trust greater financial freedoms and flexibilities and importantly enables local people, patients and staff to have a greater say in how services are provided to the local population.

Quality and Innovation
The Commissioning for Quality and Innovation (CQUIN) scheme is a Department of Health payment framework aimed to drive quality of health care. It was launched in April 2009, and is used to ensure quality is part of the commissioner- provider discussion throughout all health care organisations. The framework supports the vision set out in “High Quality for All” (Darzi) of an NHS where quality is the organising principle.

Advanced Nurse Practitioners
Advanced Nurse Practitioners are a multidisciplinary team, with the capacity to prescribe, provide a differential diagnosis, and have advanced assessment skills. They work to deliver "the right care, in the right place, at the right time".
Cardiac Nursing Service
The Cardiac Nursing Service provides support for patients diagnosed with heart failure. Care is delivered at both clinics and patient's homes.
District Nursing
Our District Nurses provide community and end of life care to the residents of Stoke-on-Trent and Staffordshire.
24 hour District Nursing
District nursing provides nursing care for patients who are housebound, or have a nursing reason that makes a home visit more appropriate.
Community Intervention Service (District Based)
Each of the eight Districts of Staffordshire has a Community Intervention Service (CIS). The CIS combines a number of different professionals from Social Care Services and the Health Service.
Community (Stroke) Rehabilitation
The Community (Stroke) Rehab Team provide care in the homes of service users.
Continence Advisory Service
This service provides specialist assessment and conservative management of bladder and bowel dysfunction within Staffordshire for all patients suffering bladder or bowel problems. The service promotes continence, provides education/specialist advice on bladder and bowel dysfunction and provides a quality and effective pad delivery service.
Changes to Continence Products
Staffordshire and Stoke on Trent Partnership NHS Trust has appointed HARTMANN to provide a comprehensive range of disposable continence care products to meet all degrees of incontinence to ensure it is getting the best quality continence products for all its patients.

The team provide a full specialist rehabilitation programme for patients who have undergone amputation surgery or have suffered congenital limb loss and require an artificial limb.

The role of a Community Hospital is pivotal to the philosophy of delivery of care closer to home and providing care for patients in the least dependant setting for their care needs. Community Hospitals include: Haywood Hospital and Walk-In Service, Longton Cottage Hospital, Bradwell Hospital, Leed Moorlands Hospital and Cheadle Hospital.

Bradwell Hospital has 3 wards and 73 beds
Cheadle Hospital
Cheadle Hospital has 2 wards with a total of 47 beds, including one supportive therapies day case bed. The hospital largely provides rehabilitation/intermediate care following an Acute Hospital admission and direct admissions from home to prevent an admission
Haywood Hospital
Formerly known as Burslem, Haywood and Tunstall War Memorial Hospital, Haywood Hospital was officially opened by Lord and Lady Harrowby on 21st October 1930. In September 2010 the Haywood Hospital was reopened to patients after major redevelopment work.

Broadfield Ward is a twenty-three bedded Specialist Neuro-Rehabilitation ward in Haywood Hospital
Haywood Hospital Walk-in Service
The Haywood Walk-in Centre is a nurse-led service which offers convenient access to a wide range of minor injury / minor ailment services. The department is open every day and no appointment is necessary.
Sneyd Stroke Rehabilitation Ward
The Stroke Rehabilitation Unit is a twenty bedded ward that provides specialist and coordinated rehabilitation to adults following a stroke and is part of a wider stroke service that includes an Acute Stroke Unit based at the North Staffordshire Royal Infirmary and the Community Stroke Team at the Haywood Hospital.

The staff provide nursing care to patients within wards, departments and our walk-in centres and minor injuries units.
Dental Services
The Dental Service provides dental care for vulnerable people and emergency or urgent dental care to anyone not currently under treatment with a dentist locally. The team consists of dentists, dental therapists, dental hygienists and dental nurses.
North Staffordshire Primary Dental Care Service
The NSPDCS provides dental care for vulnerable people and emergency or urgent dental care to anyone not currently under treatment with a dentist locally. The NSPDCS also provides specific types of care on referral from general dental practitioners.

Patient Advice and Liaison Service
The Patient Advice and Liaison Service (PALS) works in partnership with patients and staff to help improve services for our patients. PALS is part of our commitment to give high standards of care and support to everyone who uses our services.
Compliments and Complaints
We are always very pleased to hear about your experiences of our services. Please let us know about what went well, staff who you think should be praised and what you think could be done to improve services and care.
Making a complaint
Complaints can be made by: an existing or former patient, someone else with the consent of the patient (the patient's consent must be in writing), or someone such as a close relative or friend if the patient cannot complain or consent i.e. if they have died or are very ill.
Learning from complaints
It is our aim to provide you with a positive experience, no matter which of our services you are accessing. Unfortunately sometimes a service may not come up to your expectations or we may fall short of the high standards we set ourselves. All complaints will be dealt with in a positive way, being viewed as an opportunity to directly improve services for patients.
